Like all other reports and forms from Bloomwell, such as those for the driver’s license authority, the patient ID card can be easily applied for within our app with just a few clicks.

What Are the Requirements
and Costs?

After your first prescription  is issued, we offer the Bloomwell Patient ID Card as an additional proof of therapy alongside your prescription.

The card is a personalized, credit card-sized document.
The card includes your personal details on the front, and on the back, the date and place of issuance, along with a QR code containing further information and contact details of Bloomwell GmbH.

Cost: €9.01 including shipping.

To apply the Patient ID Card: Go to the "My Profile" section in the Bloomwell app, click on “Additional Services", select the “Patient ID Card” product and proceed to order it for a small fee.
